First appointment

Once you have arranged your first appointment, please fill in the patient intake form. Please also bring any relevant medical letters and reports.

Preventive check-ups U7 to U11

The preventive check-ups are used for the early detection of illness and developmental disorders and are documented in the yellow health record booklet.

Adolescent check-ups J1 and J2

The adolescent check-ups are aimed at teenagers and young adults and are used for the early detection of illness and developmental disorders, as well as a check of physical and mental health.
